Forms

We will be using the Intake and Confidentiality form.  You can either complete it during our first session, or you could bring a printed, completed copy for me to the first session. Either way, we will review the document and sign it together.   Occasionally, we will use the Release of Confidentiality Form, to facilitate communication between me and another practitioner such as your doctor.  I attach it here for your convenience, in the event that we need it.
